Up to date immunization records signed by your doctor/pediatrician must be provided BEFORE your child attends our camp program.

SUMMER CAMP PHOTO PERMISSION SLIP

During the course of our summer camp program, we like to photograph activities and the children enjoying the grounds, games, and pool, etc.

We need your permission to share their image in our camp Google Classroom so you, as their parent, are able to share in the excitement of their day.

These images will not be used for advertising or social media.

If you are not comfortable with this, we understand and will make sure not to share your child’s image.
